Registration Instructions

• All fields marked with an asterisk * are required to be filled in.
• Phone Number: It is a numeric field only. There should be no spaces, dashes or brackets. If you do not have a number for any one of the "Phone Number" fields, please complete the field with all zeros (0).
• Postal Code: There should be no spaces.
• OSA Number: This a numeric field only.
• If you intend to pay your 'Membership Fee' by cash, please print 2  copies of the 'Registration Form' prior to clicking on the submit button.  Please bring the forms, together with the relevant fee, to the Branch Education Session & hand them to the Treasurer.
• If you are paying your fees by Cheque, and would like an acknowledgment of your payment, please print the 'Registration Form' prior to clicking on the submit button.  Bring the form, with the relevant fee to the Branch Education Session & hand them to the Treasurer. Otherwise your cancelled cheque will be an acknowledgement that payment has been received.
• Your Membership will be complete after receipt of the relevant fee.

Payment Options:

1. You can hand over your payment to any one of the Executive Members at any time. Please ensure that you hand over is acknowledged.
2. Hand in your printed Registration Form together with the relevant fee to the Treasurer at the Branch Education sessions.
3. Mail in your payments to: The Treasurer, OSRA Mississauga Branch, 3980 Grand Park Dr., P.O. Box 43183, Mississauga, ON. L5B 4A7.
